By signing the RICHFIELD YOUTH SPORTS INC. (RYS) registration form for any sport and/or attending any sport, activity, or event hosted by or participated in by RYS you agree to the following: As the legal guardian(s) of the Athlete (minor child) named on page 1 you hereby give consent and approval for said Athlete’s participation in any and all activities of RYS, during the current sport season. In consideration of said Athlete being accepted to participate in RYS activities, you agree to indemnify and hold harmless RYS, it’s Directors, Officers, Managers, Coaches, Volunteers, and Agents, without regard to any negligence on their part, against any and all claims for damages, compensation or otherwise including any and all losses & expenses caused to or by said Athlete while participating in or present at activities of RYS. You further agree to return all moneys, tickets, items, equipment, etc., on time, to the proper authority or be held liable for same. You understand NO REFUNDS will be issued after an Athlete receives an RYS uniform, or portion thereof. Refunds prior to that may be prorated by the RYS Board of Directors. Participation in, or attendance at, any RYS activity requires all members and their guests to follow all guidelines and mandates that the State of New York, The County of Herkimer, The County of Otsego and Richfield Youth Sports, Inc. have in place. Failure to follow the stated guidelines/codes of conduct could result in the Coach removing you and/or your Athlete from the activity without refund. You agree RYS, RYS Sponsors, and RYS members, have permission to use and share any photos taken at RYS facilities or activities, anywhere at any time. You hereby acknowledge the contagious nature of any infectious diseases. You voluntarily agree to assume all of the risks of exposure to or contracting infectious diseases associated with participation in any RYS activities and accepts sole responsibility for any injury to themselves or their child resulting from the same. You agree to waive any liability for exposure or infection on the part of RYS, it’s Directors, Officers, Managers, Coaches, Volunteers, and Agents, without regard to any negligence on their part. You agree to hold harmless the result of any incomplete or incorrect information submitted on this form. This agreement is effective when signed and expires after 2 years. You agree not to use inappropriate language at any RYS activities. The Coach has discretion and RYS support to send you and/or your child away from any activity for any reason whatsoever, for the remainder of the activity. The RYS coordinator for your sport/activity will review the circumstances which lead to the dismissal and may extend this penalty for the remainder of the season. The RYS board may extend the ban permanently for you, your Athlete, or both. Code of Conduct - Players & Parents This document presents the expectations for conduct from athletes, families, and coaches of RYS. We hold athletes, parents, coaches, and community members to a high standard of sportsmanship. Enforcing this Code of Conduct allows RYS to promote a safe, rewarding, and fun sports experience for our entire community. We value sportsmanship, fair play, skill development, and mutual respect among all players, coaches, officials, and spectators. Athletic field/gymnasium: Food, cell phones, music devices, and other items deemed a distraction are not allowed around or in the playing areas. Equipment will not be thrown, broken, or misused (if a player breaks team equipment they will have to pay for the item). What you bring in you take out. It’s not up to the coaches to pick up after players and families. Please clean up after yourselves. Off field/court: Any action that occurs off the field/court that is deemed inexcusable and compromises the image of RYS may result in the removal of the player from the organization. ABSOLUTELY NO PETS, SMOKING, DRUGS, ALCOHOL, OR VAPING WILL BE ALLOWED!
